As the number of people living with chronic diseases has increased, so the management of health has changed, with more resource and attention directed at caring for and treating those living with long-term conditions compared to acute illnesses.
This change has consequently impacted the roles of both healthcare professionals (HCPs) and patients in terms of disease management. In the acute setting, treatment and disease management happen in a relatively short and fixed period of time, with the HCP being the expert and firmly in control of the situation.
